Hospice Medical Social Worker Bronx
DUTIES:
Provide medical social services to hospice patients and their families.
Conduct initial and ongoing psychosocial assessments to identify emotional, social, and environmental needs.
Provide individual and family counseling related to terminal illness, end-of-life concerns, and coping strategies.
Offer bereavement counseling and grief support, including follow-up with families after a patient’s passing.
Assist patients and families with advance care planning, end-of-life decision-making, and patient advocacy.
Facilitate family meetings to address care goals, emotional needs, and concerns.
Coordinate and refer patients and families to community resources (support groups, financial assistance, counseling services).
Collaborate with the interdisciplinary hospice team (nurses, physicians, chaplains, case managers) to develop and update care plans.
Participate in interdisciplinary team meetings and case conferences.
Provide crisis intervention and emotional support as needed.
Maintain timely and accurate clinical documentation in accordance with hospice and regulatory standards.
Uphold the hospice philosophy of compassionate, patient-centered, and non-abandonment care.
QUALIFICATIONS: LMSW (would consider a MSW with licence exam scheduled) 1 yr of Hospice, Palliative, Oncology exp or Acute Home care and bereavement counselling experience. Car and DL
