About This Role:
As a Community Health Nurse, you'll play a vital role in assessing, planning, implementing, and evaluating patient care plans. You'll provide direct skilled nursing care and educate patients in their homes, coordinating services with other caregivers as needed. This role offers an excellent opportunity to connect with families and empower them to achieve better health outcomes.
What You'll Do:
Assess patient health needs and develop individualized care plans.
Provide direct skilled nursing care in patients' homes.
Educate and instruct patients and families on health management and promotion.
Coordinate care with other members of the healthcare team.
Maintain accurate and timely patient records.
Travel regularly to patient assignments across NYC (public transportation and/or private vehicle).
What You'll Need:
Valid New York State RN license and current registration.
Current Basic Life Support (BLS) certification.
One (1) year of experience as a registered nurse.
Ability to travel regularly across NYC.
Commitment to continued professional development.
Preferred Qualifications:
Baccalaureate Degree in Nursing.
Two (2) years of full-time nursing experience.
One (1) year of hospital experience.
One (1) year of experience in Labor & Delivery, NICU, or postpartum care.
Knowledge of federal, state, and local home care regulations and standards.
Excellent verbal, written, organizational, and analytical skills.
Demonstrated competence in patient assessment and critical thinking.
Experience with computer software applications (Word, Excel, Access, etc.).
Experience with electronic health record systems (EPIC preferred).
