Job Description

Job Summary:

We are seeking a dedicated and compassionate Home Health Occupational Therapist to provide professional therapeutic services to patients in their homes. The ideal candidate will assess patients’ physical and mental conditions, develop individualized treatment plans, and help patients improve their ability to perform daily living and work activities.

Duties and Responsibilities:

  • Assist Occupational Therapist (OT) in conducting patient evaluations and implementing treatment plans.
  • Deliver direct therapy sessions to improve mobility, coordination, and daily living skills.
  • Educate and train patients, families, and caregivers on therapeutic exercises and adaptive techniques.
  • Monitor patient progress and report observations to the supervising OT.
  • Modify treatment activities to align with individual patient goals and abilities.
  • Document patient progress, interventions, and responses to therapy in accordance with healthcare regulations.
  • Ensure compliance with state and federal health care laws, including HIPAA and therapy practice standards.
  • Collaborate with interdisciplinary healthcare teams to ensure comprehensive patient care.
  • Assist patients with the use of assistive devices and adaptive equipment to promote independence.
  • Participative in team meetings and case reviews to provide updates and recommendations.

  • Bilingual in Spanish and English

Required Education and Experience:

  • Associate degree in Occupational Therapy Assisting from an accredited program.
  • Valid COTA license in Texas.
  • Certification from the National Board for Certification in Occupational Therapy (NBCOT).
  • Experience in clinical, rehabilitation, or home health setting (preferred).

  • Noise Level: Moderate
  • Employees work in an office environment sometimes with moderate noise levels, controlled temperature conditions and sometimes travel to patients’ homes where they may have direct exposure to hazardous substances. Employees may interact with upset staff and/or public and private representatives in interpreting and enforcing departmental policies and procedures.
  • Employees will have to travel to a variety of patient homes and perform in conditions that vary greatly depending upon the client’s home environment. Some homes will be clean, neat, and maintained at a comfortable temperature. Other homes may be cluttered, dirty, with an uncomfortable temperature.
